Privacy Policy & Terms of Use

Website Privacy Policy

Effective date: November 3, 2026
Last updated: November 3, 2026

Sleep and Sinus Centers of Georgia respects your privacy. This page has two parts. Part 1 is our Website Privacy Policy. It explains what information we collect when you visit our website, fill out a form, take a self-assessment, or call, text, or email us, and how we use, share, and protect that information. Part 2 is our Website Terms of Use.

The short version

  • We are a medical practice. Your medical information is protected by federal law (HIPAA) and by our Notice of Privacy Practices.
  • We do not sell your personal information.
  • We do not share your mobile information or text message consent with third parties or affiliates for marketing or promotional purposes.
  • We use analytics and advertising tools to understand how our website is used and how our ads perform. We do not let those tools capture what you type into our forms or your self-assessment answers.
  • Please do not send detailed medical information, insurance ID numbers, or Social Security numbers through website forms, email, text, or social media. Call us at 678-689-1100 or use the patient portal.

1. Who we are and what this policy covers

Sleep and Sinus Centers of Georgia is the trade name of Atlanta Allergy & Otolaryngology Centers, LLC ("Sleep and Sinus Centers of Georgia," "we," "us," or "our"). We provide ear, nose, and throat, allergy, and sleep medicine care at our offices in Lawrenceville and Watkinsville and at our sleep lab in Lawrenceville.

This policy applies to:

  • Our website, www.sleepandsinuscenters.com, including its forms and self-assessment tools.
  • Phone calls, text messages, and emails between you and our practice.
  • Information we collect through our social media pages.

This policy does not replace our Notice of Privacy Practices, which governs your medical records and other protected health information. It also does not cover websites and apps run by other companies, such as social media platforms, even when we link to them.

2. Your health information and our Notice of Privacy Practices

As a health care provider, we are a "covered entity" under the Health Insurance Portability and Accountability Act of 1996 (HIPAA). HIPAA protects your "protected health information," which is health information that identifies you. Examples include your medical records, test results, insurance information, and appointment history.

Our Notice of Privacy Practices explains how we may use and share your protected health information and the rights you have over it. You can get a copy at any of our offices, or call us at 678-689-1100 and we will send you one. If this policy and our Notice of Privacy Practices ever differ on how we handle protected health information, the Notice of Privacy Practices controls.

3. Information we collect

Information you give us

  • Appointment requests: your name, email address, phone number, preferred office, preferred days and times, the service you are interested in, and any message you write.
  • Contact forms: your name, email address, phone number, the topic of your question, and your message.
  • Self-assessments (such as our allergy symptom test, sinus symptom quiz, and sleepiness test): your answers and, if you choose to send them, your name, phone number, email address, and a message so our team can follow up.
  • Shadowing and observership requests: your name and contact information, school or institution, field and areas of interest, GPA, preferred dates and office, and the acknowledgments you agree to.
  • Email sign-ups and resource requests: your email address and the topics you choose.
  • Calls, texts, and emails: what you tell us, your phone number or email address, and details such as the date, time, and length of a call or message.
  • Social media and reviews: what you send us in a direct message or post on our pages.

Information collected automatically

When you visit our website, our systems and the tools described in Section 6 may automatically collect:

  • Your IP address and your general location based on it (such as city or region).
  • Your device type, operating system, and browser.
  • The pages you view, how long you stay, the links and buttons you click (including tap-to-call links), and the date and time of your visit.
  • The website or ad that brought you to our site, including an ad click identifier when you arrive from a Google ad.
  • Cookie identifiers and similar technical identifiers.

Information from other sources

We receive information from referring physicians, health plans, labs, and others involved in your care. That information is protected health information and is handled under our Notice of Privacy Practices. We also receive summary reports from our analytics and advertising providers that show how our website and ads are performing overall.

Please keep online messages brief. Our website forms are meant for scheduling and general questions. Please do not include detailed medical history, medication lists, insurance ID numbers, or Social Security numbers in a website form, email, text, or social media message. Our team will collect what we need by phone, through the patient portal, or at your visit.

4. How we use information

We use the information we collect to:

  • Respond to your requests and questions, schedule and confirm appointments, and check insurance coverage.
  • Provide, coordinate, and bill for your care, as described in our Notice of Privacy Practices.
  • Send appointment reminders, recall reminders, pre-visit and pre-procedure instructions, patient portal notices, and billing messages.
  • Tell you about our services, events, and practice news by email or mail where the law allows, always with a way to opt out.
  • Review and schedule shadowing and observership requests.
  • Run, secure, and fix our website, and block spam, bots, and fraud.
  • Understand, in summary form, how visitors use our website and which ads lead people to call or request an appointment.
  • Train our staff and improve our service.
  • Meet our legal and regulatory obligations, respond to lawful requests, and protect the rights, safety, and property of our patients, staff, and practice.

5. Self-assessment tools

Our online self-assessments are educational tools. They are not a diagnosis, and they do not replace an exam by a licensed provider. Your answers are only sent to us if you submit the form.

If you submit your answers with your name or contact information, our team uses them to follow up with you and to prepare for a possible visit. We treat that submission as protected health information under HIPAA and our Notice of Privacy Practices. We do not send your answers to analytics or advertising providers.

6. Cookies, analytics, and advertising tools

Cookies are small files a website stores in your browser. Tags and pixels are small pieces of code that send information to a service when a page loads or when you take an action. We use these technologies to keep our website working, to keep it secure, and to measure how it and our advertising perform.

Tools we use on our website

  • Google Tag Manager: loads and manages the Google tools listed below.
  • Google Analytics: measures visits, pages viewed, and actions such as tapping to call or submitting a form, and reports them to us in summary form. It uses cookies such as "_ga."
  • Google Ads conversion tracking: tells us whether a Google ad led to a phone call, a form submission, or an appointment request, so we can measure and pay for our advertising. It uses cookies such as "_gcl_au" and stores the ad click identifier from the ad you clicked.
  • Cloudflare Turnstile: checks that form submissions come from a real person and not a bot. It processes your IP address and technical information about your browser.
  • YouTube: plays the videos embedded on our pages. YouTube, which is owned by Google, may set cookies when a page with a video loads or when you play a video.
  • Webflow: hosts our website. Webflow and Google Fonts receive your IP address and basic technical information when they deliver our pages and fonts to your browser.

Google, YouTube, Cloudflare, and Webflow may collect information about your activity on our website and on other websites over time. Their use of that information is governed by their own privacy policies.

Our rules for these tools

  • We do not let analytics or advertising tools capture what you type into our forms or your self-assessment answers.
  • We do not place our analytics or advertising tools in the patient portal or in our online scheduling system.
  • We do not use your health information to target advertising to you, and we do not build advertising audiences from people who visit pages about specific conditions or treatments.
  • We do not sell information collected by these tools.
  • If we add a tool that changes what information is collected, we will update this policy first.

Your choices

  • Browser settings: you can block or delete cookies in your browser settings. Some parts of our website, such as forms, may not work without them.
  • Google Analytics: you can install the Google Analytics opt-out browser add-on at tools.google.com/dlpage/gaoptout.
  • Google ads: you can manage ad personalization at myadcenter.google.com.
  • Do Not Track: our website does not currently respond to "Do Not Track" browser signals. You can use the choices above instead.

7. How we share information

We do not share consumer data except in the limited circumstances listed below, where sharing is necessary to provide services to you, to operate our practice, or to comply with legal requirements. These circumstances are also described in our Notice of Privacy Practices.

  • Service providers who work for us. These include our electronic health record, patient portal, and scheduling vendor; our website host and form processor; our phone, text messaging, email, and IT providers; and our billing, payment, and marketing service providers. They may use the information only to provide services to us. Any provider that handles protected health information must first sign a business associate agreement with us, as HIPAA requires.
  • People involved in your care and payment, such as other providers, your health plan, and anyone you authorize, as described in our Notice of Privacy Practices.
  • When the law requires or allows it, such as to respond to a court order or subpoena, for public health and safety, to government regulators, or to prevent a serious threat to health or safety.
  • Business changes. If our practice is merged, sold, or reorganized, information may transfer to the new owner, subject to HIPAA and this policy.
  • With your permission.

We do not sell, rent, or trade your personal information. We do not use or share your protected health information for marketing without your written authorization, except as HIPAA allows.

Privacy and data sharing for text messaging

  • We do not share mobile information or SMS consent data with third parties or affiliates for marketing or promotional purposes.
  • Text messaging originator opt-in data and consent are excluded from every category of sharing listed above. This information will not be shared with any third party.
  • Consumer personal information collected through our text messaging program, including SMS consent data, will not be sold, rented, or transferred to external organizations.

8. SMS Terms and Conditions

By opting in to receive SMS messages from Sleep and Sinus Centers of Georgia ("Sleep and Sinus"), you agree to receive conversational messages related to your interactions with our team. We use our text messaging program only to reply to and continue conversations that you start with us. We do not send marketing or promotional messages through this program.

  • How to opt in: start a text conversation with us at 678-689-1100.
  • Message frequency: message frequency may vary. On average, 1-2 messages per month.
  • Cost: message and data rates may apply.
  • Help: reply HELP or INFO for assistance, or call us at 678-689-1100.
  • Opt out: reply STOP or UNSUBSCRIBE at any time. You will receive one message confirming that you have been unsubscribed, and you will receive no further messages unless you opt in again. You may also opt out by telling us in any other reasonable way, such as by phone or in person, and we will honor your request promptly and no later than 10 business days after we receive it.
  • Consent: opting in to text messages is not a condition of any purchase or of receiving care.
  • Carriers: carriers are not liable for delayed or undelivered messages.
  • Privacy: see Section 7 above and this Privacy Policy at https://sleepandsinuscenters.com/privacy-policy.

Text messages are not encrypted. Please do not text us detailed medical information, insurance ID numbers, or Social Security numbers. If you change or give up your mobile number, let us know so messages do not go to someone else.

9. Email and phone calls

Email. Regular email is not a secure way to send health information. We may send health information by secure, encrypted email or through the patient portal. If you ask us to use regular email, we may do so after explaining the risks. Our marketing emails include an unsubscribe link, and we honor unsubscribe requests promptly.

Phone calls. Calls to and from our offices may be monitored or recorded for quality, training, and record keeping. Information you give us over the phone about your health or care is protected health information.

10. Patient portal and online scheduling

Our patient portal and online scheduling tools are provided for us by our electronic health record vendor, eClinicalWorks, including its healow platform. Information you enter there becomes part of your medical record and is protected under HIPAA and our Notice of Privacy Practices. Those tools also have their own terms of use and privacy policies. Please keep your portal username and password private, and log out when you use a shared device.

11. How we protect information

We use administrative, physical, and technical safeguards designed to protect the information we hold. These include role-based access so staff see only what they need for their job, regular HIPAA privacy and security training, encrypted (HTTPS) connections to our website, written agreements with vendors who handle protected health information, and ongoing review of our systems. No website, email, or electronic system is completely secure, so we cannot guarantee that information sent to us online will never be intercepted or accessed without authorization.

If a breach affects your protected health information or personal information, we will notify you and the appropriate authorities as required by the HIPAA Breach Notification Rule and Georgia law.

12. How long we keep information

  • Medical records are kept for at least as long as Georgia law and our record retention policies require.
  • Website inquiries that do not become part of a medical record are kept only as long as we need them to respond to you and for legitimate business, legal, and compliance purposes, and are then deleted.
  • Shadowing and observership requests are kept as long as needed to review and schedule them and as our records policies require.
  • Analytics and advertising data are kept according to the retention settings we choose in those tools.

13. Your choices and rights

Information collected through our website

You may ask us to tell you what personal information we have collected about you through our website, to correct it, or to delete it, unless we are required to keep it by law or it has become part of your medical record. You may also opt out of text messages and marketing emails at any time, as described in Sections 8 and 9. To make a request, contact us using the information in Section 18. We will need to confirm your identity before we act on a request, and we will not treat you differently for making one.

Your rights under HIPAA

Our Notice of Privacy Practices describes these rights in full. In summary, you have the right to:

  • See and get a copy of your medical records, including in electronic form.
  • Ask us to correct your medical records.
  • Get a list of certain disclosures we have made of your health information.
  • Ask us to limit how we use or share your health information, including asking us not to share information with your health plan about care you paid for in full yourself.
  • Ask us to contact you in a certain way or at a certain place, such as a different phone number or address.
  • Get a paper copy of our Notice of Privacy Practices.
  • Be notified if a breach affects your unsecured health information.
  • File a complaint with us or with the U.S. Department of Health and Human Services. We will never retaliate against you for filing a complaint.

Residents of other states

Some states give their residents additional rights over their personal information. If you live in one of those states, contact us and we will respond as your state's law requires.

14. Children's privacy

Our website is not directed to children under 13, and we do not knowingly collect personal information online from children under 13. Parents and legal guardians should contact us on behalf of minor patients. If you believe a child has sent us personal information through our website, contact us and we will delete it.

15. Links, social media, and reviews

Our website links to websites we do not control, including social media platforms, review sites, and the patient portal and scheduling sites of our vendors. Those sites have their own privacy policies, and we are not responsible for their practices.

Anything you post publicly on our social media pages or on a review site can be seen by anyone. To protect your privacy, we will not confirm that you are a patient or discuss your care in a public reply. Please call us or send a private message, and we will follow up directly.

16. Accessibility

We want this policy to be available to everyone. If you need it in another format or language, or need help understanding it, call us at 678-689-1100. You can also read our Accessibility Statement.

17. Changes to this policy

We may update this policy from time to time. When we do, we will change the "Last updated" date at the top of this page. If we make a material change to how we use or share personal information, we will post a notice on our website before the change takes effect and give any other notice the law requires. If we change our Notice of Privacy Practices, we will make the new notice available on this website and in our offices.

1. Not medical advice

The content on this website, including articles, videos, and self-assessments, is for general education only. It is not medical advice, and it does not replace an exam, diagnosis, or treatment by a licensed provider. Using this website, submitting a form, or sending us a message does not create a provider-patient relationship. Always talk with a qualified provider about your own health, and do not delay or ignore medical advice because of something you read here.

If you have a medical emergency, call 911 or go to the nearest emergency room. Do not use our website forms, email, text messages, or social media for urgent or emergency needs.

An appointment request sent through this website is not a confirmed appointment until our team contacts you to confirm it.

2. Self-assessments and results

Our self-assessments are screening and education tools, not diagnostic tests. Patient stories and testimonials on this website reflect individual experiences. Results vary from person to person, and no particular outcome is guaranteed.

3. Use license
